ASSESSMENT INFORMATION Assessment Title Assessment Task Two – Written Assignment             Purpose This assessment will provide students with an opportunity to engage with a health consumer, to gain insight into the impact chronic illness and/or disability has on the consumers life, and to plan person-centered
