Height= 1.58m, weight 48kg. GP letter stated that patient weight was recorded as 60kgs 3 months ago.

Patient D.
Child. Male . 10 days old. Health visitor review. Normal delivery. Good AGPAR scores. Breast fed. Not feeding well in last 24hrs. He has not yet regained his birth weight. Tommy has passed urine once in the past 8 hours. He last had his bowels open 12 hours ago. Tommy isn’t feeding as expected, mum is waking him for feeds, only latching for a few minutes at a time before becoming tired.

Concerns that patient was not eating and neglecting herself and the home environment and has been reluctant to seek help and support.
