In alignment with your literature review, you will need to complete a care plan for your chosen case study (you will need to complete the care plan for the same case study as for which you undertook your literature review).
You should complete your care plan using the care plan template provided to you by your subject coordinator.
Whilst undertaking your care plan, you will need to identify:
- The health needs (physical, psychological, social, and environmental) of your case study on admission, including into the sub-acute (rehabilitation) environment. What supports would they likely need? What referrals would they need? This may include to allied health professionals whilst they are inpatient, or for discharge planning after they return home.
- Identify how a collaborative and person-centered model of care could enhance the health care outcomes of your person
- Discuss the role of the registered nurse to support care of the person, ensuring a person- centred approach is maintained throughout their rehabilitative stay, from admission to discharge.
Learning Outcomes Assessed:

- outline the models of care used in rehabilitation nursing in the Australian health care context.
- examine the process of rehabilitation and collaborative management of persons with injury, physical disability, and chronic illness.

Outline of Assessment No: 4 Details for the submission of Care Plan
NOTE: The following is important for students to understand and complete Assessment No: 4.
All students will need to ‘USE’ and follow the care plan template. This template has been designed to guide students in what is needed for this assessment.
- Approximate word guides have been given to help you with placing the structure of this assessment together.
- Fill in the details according to the care plan outline given (as seen below)
· Case Study No:
- Name of the Patient:
- Introduction to the patient: (100 words approx.). This will let the reader obtain a brief background to the patient, where the patient is, and why?
- Discuss the process of rehabilitation and the collaborative management of people with injury, disability, chronic illness as applied to your chosen case study… You need to explain what the process of rehab will be for your chosen patient and how or what members of the team will need to be involved in rehabilitation of your patient. (Referenced) (250 words approx.)
Now it’s time to follow the Care Plan Template as set out below:
- There is approx. 400 words for each of the ND, interventions, rationales, S.M.A.R.T. goals and evaluation. For both ND’s etc = 800 words.
- You need to have 2 x Nursing Diagnosis (ND) for this care plan. Make sure you choose a ND and NOT the doctor’s diagnosis that the patient has been given. (The example shown in the template is just an ‘example’ of how to establish a ND, but it cannot be used).
- For each ND you will need to come up with at least 2 or more x interventions. These interventions will need to be referenced, as they need to be in line with best practice as a nurse in the rehabilitation setting. You also need to make sure they will match with the Model of Care you have decided to work in with your patient.
- Each intervention needs to have: Specific, Measurable, Achievable, Realistic & Timely goals. This way you will see that what you have put into place for the patient, is measurable and realistic with a time frame.
- Now finally, you will evaluate if the interventions and goals you put into place have worked. You need to state how you evaluated the patient to see if the intervention worked. (What methods did you use? Etc.…)
- Now the Care Plan has been completed.
· Discuss which model of care you have chosen to use for your patient, to implement the care plan and why (Referenced) (250 words approx.).
- Now you will need to add a brief conclusion to end the assessment to make it complete. This is just a summary, remember do not add any think new into this part it is all about the rounding off to a good finish for the reader of this assessment.
