Case Scenario
You are a Registered Nurse (RN) working in the Emergency Department of a regional hospital in the Blue Mountains area when Mr David Parker, a middle-aged man, is brought in by ambulance, accompanied by his wife. Mr Parker is sitting upright on the ambulance trolley with an oxygen mask over his pale, grey face. He is very short of breath, anxious and complaining of pain in his chest.
The doctor examines him and orders a stat dose of IV Morphine. David’s ECG is attended and shows ischaemic injury (ST elevation) in the anterior leads. It is confirmed that David has had an acute myocardial infarction (AMI). After being admitted to the Cardiac Unit David becomes increasingly agitated, yelling at staff to contact his children and wanting his wife to take her home. David’s children are brought in to see him, however he becomes increasingly agitated and tells the children he is alright and to return to school. His takes takes them home and returns later that evening.
It becomes clear that David has experienced further heart tissue damage, with associated pain. As he starts to recover you create a care plan for David based on short term care goals and guided by relevant aspects of the heart foundation resource. You reflect on what to communicate to David’s wife when she returns to the hospital and how to include his three children in the conversation.
