Part A: Individualised Plan for three clients with complex needs
Client 1
The first client currently has a disability related to hearing impairment, and 70% of the client’s hearing or no longer used, and due to this, the client is facing severe challenges in real life. For an individualised plan, the incorporation of innovative tools has been incorporated with the incorporation of a good audiologist who can help to properly manage the hearing loss journey through the implementation of contemplation. As coined by Swanepoel et al. (2019), it is very important to help the person by giving them proper attention and speaking face-to-face and slowly. The person will also be provided hearing aids which will help the person to listen more precisely through the hearing aid device and reduce the hearing complexity from the person.
Client 2
This client is going from severe impairment disability and is currently in the 90% visual impairment stage, as well as both of the eyesight could not be corrected to a normal level as suggested by the doctor. The client needs emotional support and mental support as well as educational support to properly read and write and also can conduct daily life activities by using other body parts. As coined by Olusanya et al. (2019), it becomes very much important to give the person training to read braille scripts so that the person who defectively read and write and also social and emotional support needs to be provided for further implementation of strategies.
Client 3
The third client had a locomotor disability, and this client could not make movement from one place to another place without getting help from a third person. This client mainly has a disability in lakhs, and it is also related to the bones and joints as well as muscles of the lower body part and causing problems in the movement of a person like walking and picking or holding up any things in hand. Aids and assistive devices are required, and a wheelchair specifically needs to be provided to this client to improve the quality of life and better movement capability (Pant et al. 2018). Proper directions also needed to be given after taking feedback from doctors in terms of medication and physiotherapy in the lower body part.
Part B: Evaluation Report for three clients with complex needs
Client 1
Delivery
For the first client, it is very much important to incorporate solutions for hearing disability, and it has been identified that providing interpreter services is very much useful to use as a mode of communication. Interpreter services also include assisting learning for people currently having a hearing impairment, and it also incorporates a manual form of coded English and signs language resources. Another important thing is to incorporate assistive technology to support their communication and rehabilitation for hearing enhancement.
Monitoring
For supporting the hearing impairment of client, assistive support will be provided under the supervision of an audiologist. The people currently incorporated in the healthcare facility will be responsible for monitoring the program, and Dr. James will be responsible for giving proper treatment as he has all the workability to deal with the situation. The client was very much happier, and from his observation, the client needs some time to properly adopt the assistive support.
Evaluation
It has been identified that the program is successful as the family and team members of the client are also very much happier. The outcome of the plan is successfully achieved, and the program is able to meet the needs of the clients.
Review
This program directly caters to the needs of the clients, and it was also evaluated by continuous improvement strategies, which is very much important for further development. Yeah, the major strength of this program is that it successfully helps people facing hearing disabilities to cater to their needs; however, it requires some technologies for properly helping the clients in the near future.
Client 2
Delivery
This client is facing challenges with impairment disability, and for this client community rehabilitation programme has been implemented in which the client will spend time with other visually impaired persons to identify their living habits and daily life activities. The client will also be taken to different I hospitals to check the current condition, and after that, a proper rehabilitation programme will be implemented for providing social and psychological as well as emotional counseling and assistance. Proper training will be provided to the client, including social classes and mobility, as well as communication and also daily living skills, with some special provisions as needed by the client.
Monitoring
The monitoring plan is mainly identifying the effectiveness of the client’s community rehabilitation program, and it has been identified that Mr. Gupta will be responsible for monitoring the plan. The client was very happy during the program implementation.
Evaluation
It has been identified that the family and team members were very happy after seeing that the client is also happy in this rehabilitation programme. It also has been identified that it is very much helpful for them as the client, under the supervision of doctors and other medical staff, was providing a high level of care and assistance for solving the visual impairment-related problems further; it is also beneficial for the client to live their life at the fullest. And the major outcome of the plan is to provide proper support to the client, which is successfully achieved. This program is also Catering to the needs of the client for a long time and will give the client the proper support to live daily life. Direct feedback methods were used in the evaluation of the program.
Review
From the review, it has been identified that the plan was successful; however, as the client is nearly 90% visual impairment and does not have any hope of restoring the eyesight, therefore, the strategy is only 50% useful for the client. It will also be very much important to include more medical staff and give proper training to the client regarding braille learning so that the client can easily read and write the letters and subjects in the future without making problems for their people. It will also be very much important to implement continuous improvement strategies as the family and team members are facing a challenge to make proper communication with the client in the future.
Client 3
Delivery
To support this client, assistive services will be provided that will take the distribution of wheelchairs and crutches as well as a walker that can be tagged to the client and help in their mobility. Locomotor disability is one of the deadly diseases that can cause the malformation of the boom, therefor providing assistive support are very much important. Another rehabilitation plan also has been included in which special health care providers will come and give their entire support through physical and psychological counselling to allow the clients to live a better life and they can easily manage the disability and can live with it for a long time.
Monitoring
Monitoring it has been identified the client was very happy after getting the service, and most of the healthcare-related people are responsible for properly monitoring the plan. It has been identified that the client successfully identified the purpose, and he also wanted to include himself in future scenarios so that he can help other people who are facing challenges with their locomotor disability and give better hope to live a better life for a long time.
Evaluation
From the evaluation, it has been identified the team members were very happy to see that client is making a good level of preparation and also successfully handling the pressure of locomotor disability. The family members were also very much happy to see that the client is making a good amount of progress and do not face many challenges in living life. Direct communication has been made, and an online meeting has been conducted to take feedback from the client directly to properly evaluate the program.
Review
It can be seen that the program is successful, and a major strength of this program is the high engagement from the client. The major weakness of this program is that it could not be helpful for clients who have had locomotor disabilities from the very first part of their life, and it will be challenging for the healthcare professionals to give proper support to the clients.
