PORTFOLIO ASSESSMENT 3

PORTFOLIO ASSESSMENT 3

You will be creating a Portfolio record specifically focused on mental health assessments and pharmacological interventions. This is designed to assist in the development of theoretical and clinical skills in mental health, with a direct connection to your clinical practice as a registered student nurse. The evidence record provides you with the opportunity to showcase your learning and display completed work undertaken in this unit.

The evidence record will include two artefacts. An artefact is a piece of work that supports the development of theoretical and clinical skills. The artefacts relate to three core clinical areas; assessment; care planning; and pathophysiology and pharmacology. These artefacts will allow you to demonstrate a beginning level of understanding of using assessment and evidence to plan recovery-focused care in partnership with mental health consumers.

Key Assessment Information
Portfolio: Completion of an evidence record specifically focused on mental health  Artefact 1: Comprehensive mental health assessment (1000 words) Artefact 2: Medication diary (1000 words)2000 words 40%Sunday 23:59 hours of Week 3

 PURPOSE

The purpose of this assessment task is to:

  • Develop an Portfolio evidence record.

This provides an opportunity to apply and develop mental health, theoretical and clinical knowledge and skills. The evidence record requires that you reflect on the allocated clinical scenario and further develop your skills and knowledge in how to conduct comprehensive psychiatric assessments and enhance medication knowledge for people diagnosed with anxiety, mood and psychotic disorders. The assessment frameworks that are described in this unit support recovery-oriented and trauma-informed care. This assessment enables you to record your learning and curate a series of artefacts that will form an Portfolio of evidence of your learning in this unit. This evidence record can be utilised for future units where mental health is emphasised and can be provided as an addition to your resume.

 LEARNING OUTCOMES

  1. Review how the regulatory frameworks, standards and codes of practice and ethics, and government policies guide the safe delivery of evidence based person-centred mental health care;
  2. Articulate knowledge and skills in psychiatric assessment, planning, implementation and evaluation and documentation of care to formulate nursing management of people with selected mental health conditions across the lifespan using digital health;
  3. Appraise population health and nursing-related theories of mental distress to examine mental illness across the lifespan;
  4. Contextualise clinical reasoning and critical thinking when planning and implementing recovery oriented, trauma informed nursing care to people with anxiety, mood and psychotic disorders across the lifespan;
  5. Critical reflect on their professional and therapeutic communication using the principles of partnering with consumers and carers and taking into account Aboriginal and Torres Strait Islander peoples, other cultures and people of diversity.
  6. Exhibit knowledge in pathophysiology and the principles of pharmacodynamics and pharmacokinetics in psychotropic medication for the pharmacotherapeutics and quality use of medicines.

 ASSESSMENT DETAILS

For the Portfolio you will develop two artefacts/assessment pieces that directly relate to the clinical scenario below which is in two parts. Watch both videos below:

Clinical Scenario Part A opens in new window

Clinical Scenario – Part B opens in new window

The Two Artefacts/Assessment pieces include the following:

  • Comprehensive mental health assessment: Including a Mental State Examination and Risk assessment (1000 words)
  • Psychopharmacology – Medication diary. Choose one medication from each of the Four classes of psychotropic medications: anti-anxietyantidepressantmood-stabilisingantipsychotic. Complete the medication diary template. (1000 words)

Considerations for artefacts/assessment pieces:

In the Portfolio you will need to:

  • Use relevant standards and evidence-based research to support your work
  • All assessment pieces must be completed comprehensively and holistically
  • Ensure that you use a recovery-oriented and trauma-informed approach to planning nursing care
  • Demonstrate an understanding of the pathophysiology of mental disorders and its relationship to psychopharmacology and quality use of medicines
  • Ensure that all information that you have sourced is only from peer reviewed journal articles or appropriate textbooks and is correctly referenced with APA 7th edition for citations and reference list

Artefact 1: Comprehensive mental health assessment: including a Mental State Assessment and Risk Assessment

This artefact will demonstrate your:

  • knowledge and use of structured assessments and the range of tools that support clinical assessment.
  • ability to conduct a comprehensive psychiatric assessment, mental state examination and risk assessment.
  • ability to accurately record observations using assessment frameworks.

Artefact 2: Psychopharmacology and Pathophysiology – medication diary

This artefact will demonstrate your understanding and provide a resource for the psychopharmacology and pathophysiology of mental disorders.

For this artifact, discuss and explore the following points: Document findings using the medication diary template.

You must:

  • Use GENERIC MEDICATION NAME (and give alternative brand names) & DRUG CLASS (include drug schedule)
  • Identify and describe the psychotropic CLASSIFICATION groups, (anti-anxiety, antidepressant, mood stabilising, and antipsychotic) and the psychiatric disorders for which they are commonly used in psychiatry.
  • ACTIONS (link to pathophysiology) & INDICATIONS

Describe the pathophysiological responses and effect of psychotropic medication for a person with a mental health disorder.

  • ADVERSE REACTIONS (most common) and INTERACTIONS (including drug interactions if any)

Identify side effects and precautions related to chosen psychotropic medication class.

  • METHODS OF ADMINISTRATION

Consider safe administration of medication, daily dosage range, route of administration and relevant legal and ethical issues.

  • NURSING CONSIDERATIONS/CONSUMER EDUCATION

What is the role of the nurse in the administration of psychotropic medications and related interventions? Consider safe medication administration; management of interactions and side effects; promoting medication adherence; consumer education; assessment and monitoring of physical and mental health; consumers’ and carers’ perspectives including the lived experience research relating to the medication.

All written assessments must align to the following academic standards:

  • use high-level quality contemporary evidence-based literature to inform the discussion and critical analysis, date of evidence within ten years;
  • use of the third person, unless specifically indicated otherwise;
  • APA 7th edition for in-text citations and reference list;
  • adherence to the word count; ten percent allowed either side of word count;
  • a 12 size font in either Arial, Times New Roman or Calibri;
  • 1.5 line spacing; and submitted using an MS word document only.

All assessments should include:

  • Unit Code & Unit Name
  • Student Name
  • Student Identification number
  • Title of assignment
  • An accurate word count that includes in-text citations, but excludes the reference section at the end of the essay

Attend a writing drop-in session with a learning advisor for help with planning your assessment or feedback on a draft. VU Learning Hub opens in new window

Students should refer to VU Assessment for learning policy regarding word count and late submission penalties.
