Aim
This task requires you to demonstrate your understanding of key concepts presented in this unit. In module 2 health promotion, epidemiology and health priority areas are introduced. The concept of supporting people to manage chronic conditions is explored in module 3. Integrating these with the principles of primary health care and primary health care nursing practice consolidates unit content.
As with all nursing care, a response to a health concern should be based on understanding the underlying contributing factors, planning an appropriate intervention and evaluating the result of the action taken. This tasks requires you to adopt this process and apply to population groups rather than individuals.
Task description
To complete this assessment task you need to:
Identify one health issue/concern and provide justification using epidemiological data to explain why this issue/concern is a national health priority.
Analyse the contributing factors for this health issue/concern and map out a health promotion plan to potentially address or mitigate these. A minimum of 2 objectives and 2 strategies per objective are required.
Select one potential health promotion intervention/activity you have identified to describe in detail, including how evaluation of this intervention could be undertaken.
Include a minimum of 5 academic references.

Task length: 1000 words (+/- 10%).

Assessment criteria and marking rubric
Your submission will be assessed against the following criteria:
Epidemiological data is used to explain why the health issue/concern identified is a National Health Priority (20%). Factors that contribute to the health issue/concern identified are analysed, with consideration of relevant social determinants, to develop a health promotion plan in response that will improve health equity** (40%).
A detailed summary of a proposed health promotion intervention activity is described, including details of how this activity will be evaluated (30%).
Adhere to academic presentation requirements (10%).
**Please note that for this task you do not need to explain how your health promotion plan will improve health equity! Focus on the plan itself. By addressing the health need and considering relevant social determinants, you will improve health equity.
