
STUDENT INSTRUCTIONS
You are required to write a report as outlined in the assessment instruction and criteria listed below. It is important to ensure you read all aspects of the assessment topics and discuss any areas that require clarification with your assessor.
Where there is a word limit it is important to be aware of this parameter although regardless of the word count all criteria must be covered to the required standard.
- This is an individual assessment.
- If you feel that you may be entitled to access reasonable adjustment strategies to assist you in completing the assessment, you should discuss the issue with your assessor before the assessment is due.
- If an extension is required, please request it in writing at least three (3) days prior to the assessment taking place.
- You have two (2) attempts to achieve a satisfactory result on each part of the assessment.
- This assessment will be conducted in the student’s own time.
For this assessment, you are required to cover all assessment criteria listed below to the standard required to achieve a result of satisfactory. All assessments must be assessed as satisfactory for you to be deemed competent in this unit.

ASSESSOR INSTRUCTION Students must be provided with clear and concise information in relation to the Report criteria and how this criteria relates to the specific unit of competency. Students must have a clear understanding of the intent behind the assessment task prior to commencing the task. Where a word limit is applied it is important to be aware that regardless of the word count all criteria must be covered to the required standard and the assessment tool must clearly measure each required point. Please note that although you may set a standard of presentation for your Report submissions this cannot be a criteria that contributes to competency for the unit. Remind the student that they have two (2) attempts to achieve a satisfactory result on each part of the assessment. If incorrect answers are orally clarified or the learner is provided with time to review incorrect answers, this must be clearly identified in the right-hand column by the assessor and initialled, and this is considered as one (1) attempt. A record of original attempt/re-attempt must be maintained by the assessor and kept on the student’s file. |

This is no longer only happening in high-income countries. Four out of five chronic disease Deaths today are in low and middle-income countries. People in these countries tend to Develop diseases at younger ages, suffer longer – often with preventable complications – and die sooner than those in high income countries.” (WHO, 2005) Assessment: For this assessment, you are to write a word submission demonstrating your Knowledge of the context for chronic disease. Requirements for Submission Word limit 1500-2000 words APA Referencing style 7th Edition to be used .The assignment must be submitted electronically via the MOODLE dropbox. Your submitted file must be named using the following format: – E.g. Name_Surname_St. No_GroupBJ1234_Assessment 4_HLTENN012. Students must keep a copy of all work submitted.Assignments marked Not Satisfactory must be handed in within 2 weeks of the student receiving it back. The maximum mark for a resubmitted assignment is Satisfactory. The original marked copy must be resubmitted with the revised version.All reference and source materials must be accurately referenced where the information is unsubstantiated and not produced by a recognised and reputable organisation or published author.Plagiarism is not acceptable. The laws of copyright mean that you cannot copy, or cut and paste text from resource material. The assignment must be in your own words and must be referenced appropriately when you have taken your ideas from another source. Direct quotes must be kept to the absolute minimum and must be referenced appropriately.
